Helping primary care providers support their patients with complex needs

Caring for patients with intellectual and developmental disabilities often requires specialized expertise and strong collaboration across providers. That’s why, at Surrey Place, our physicians and nurses partner with primary care providers to offer consultative support for children, youth, and adults with complex needs.

Primary care providers often refer to us when their patients need these types of support:

  • Consultations for complex medical and developmental presentations
  • Mental health concerns and medication management
  • Behaviour or psychiatric recommendations

How to refer

To be eligible for these services, clients must be referred by a healthcare provider or Surrey Place clinician.

Referrals can be made via fax  or through the Ocean e-referral platform.

To be eligible for these services, clients under 18 must be:

  • Registered at Surrey Place
  • Referred by a healthcare provider (e.g., physicians, nurse practitioners) or Surrey Place clinician to access all clinics under Nursing

To be eligible for these services, adult clients must be:

  • Registered with Developmental Services Ontario (DSO)
  • Referred by a healthcare provider (e.g., physicians, nurse practitioners) or Surrey Place clinician to access all clinics under Nursing
