Who we support
Service Coordination supports people of all ages with intellectual or developmental disabilities. Support may look different depending on age, needs and eligibility. Children and youth may be connected to Coordinated Service Planning, while adults may be connected to adult service coordination through Developmental Services Ontario.

Our dedicated team of staff will explore your concerns and discuss your individual development and strengths.
Service guidance
Once we’ve assessed your needs, you’ll receive connections to assessments and services. Our team uses an interdisciplinary approach to coordinate with service team members, day supports, and other community supports and to collaborate with other service providers across sectors.
We make sure you find the right support services
The number of services available to people with intellectual and developmental disabilities can feel difficult to navigate. Service Coordination acts as a hub to help people and families understand what supports may be available and how to access them.
Depending on your age, needs and eligibility, our team may help connect you to Surrey Place services, Coordinated Service Planning or other community supports. We can also help you understand referral pathways and next steps, so you know where to start.
-Infants 0-6 years of age must have a supposed or confirmed developmental disability
-Children and youth aged 6-18 years of age must have a confirmed intellectual disability
-Adults 18 years of age and older must be registered with Developmental Services Ontario Toronto Region
