Who we support

The Family Medicine Consultation Clinic supports adults aged 18+ who have one or more intellectual or developmental disabilities (e.g., autism, FASD, complex physical or mental health issues, undiagnosed developmental disorders).

Assessments

We’ll help you schedule your specialized assessments, which may include health history, physical exam, and safety concerns.

Care plan

When the assessments are complete, we’ll develop a comprehensive care plan. With consent, we can share this report with primary care providers and the client’s circle of care—so everyone feels supported and on the same page.

Helping you enhance adult healthcare

Our Family Medicine Consultation Clinic provides specialized support for adults with intellectual or developmental disabilities. Once you have received a referral from your primary care physician, this service provides tailored medical guidance, including health assessments and clear recommendations to ease the stress of coordinating care.

To be eligible for this service, clients must be registered with Developmental Services Ontario and have a referral from their primary care provider sent to Surrey Place.

We work with adults who have one or more intellectual or developmental disabilities, which may include autism, FASD, cerebral palsy, mental health disorders, complex physical or mental health issues, or other undiagnosed developmental disorders.

The most common medical concerns we support investigating the link between physical or mental health and challenging behaviours, such as self-injury or aggression.
