Extended Needs Services is for children and youth aged 0–18 with complex needs such as:

-Neurodevelopmental conditions

-Acquired brain injury

-Mental health conditions

-Co-occurring chronic physical health conditions

-Caregiver/family complexity

Assessment

Our dedicated team of staff will explore your concerns and discuss your individual development and strengths.

Service guidance

An appointed team lead will coordinate communication and collaboration based on your family’s priorities. This ensures that client records are shared among all their teams, reducing the need to repeat the same information to different service providers.

Rapid support for children with extensive needs

The Extensive Needs Services provide timely support for children with urgent and extensive medical, health, developmental, and social needs. This program is delivered by Holland Bloorview, the Children’s Hospital of Eastern Ontario, and McMaster Children’s Hospital in partnership with Surrey Place.

Our dedicated Service Coordinators will:

  • Help you determine your goals for services
  • Make referrals to the right community supports
  • Ensure your family is at the center of the decision-making process
  • Explore your concerns and discuss your child’s development and strengths
  • Provide streamlined connections to coordination of services
  • Provide transition support and ensure your family is prepared for discharge

To be eligible for this service, clients must be:

-Child or youth aged 0-18

-Children or youth with complex profile of needs (e.g., neurodevelopmental conditions or acquired brain injury, mental health conditions, and co-occurring chronic physical health conditions)

-Have safety concerns (self or others) or significantly impaired functioning

-Have caregiver or family complexity (e.g., finances and financial pressures, language or cultural barriers, family capacity, or living arrangements)

-Live in the GTA
